A federal report shows a fatal accident at an underground coal mine in Harlan County happened because the mine operator did not effectively prevent the dangers of falls from pillars of rock and coal.Our news partner, The Herald Leader, reports the accident occurred on May 22 at the Rex Coal Company’s CVB No. 1 mine.A 2,000-pound chunk of coal and rock fell on Felix Matthew North, 48, of Bell County killing him. North was moving a power cable for the machine when the piece fell on him.
